I, ______________________________, hereby acknowledge and agree that I have voluntarily chosen to participate in the dance program sponsored by Live to Dance • Dance to Live [a 3Wong.com division] ("Organization"). In consideration of being accepted into the program and receiving tuition coverage as a sponsorship, I hereby agree to the following terms and conditions:

1. Assumption of Risks: I understand that participating in dance activities may involve certain risks and hazards, including but not limited to physical exertion, potential injury, and the risk of accidents. I acknowledge that I am voluntarily participating in the dance program, and I assume all risks associated with my participation.
2. Medical Fitness: I certify that I am in good health and physically capable of participating in the dance activities sponsored by the Organization. I understand that it is my responsibility to consult with a medical professional if I have any concerns about my ability to safely participate.
3. Release of Liability: I hereby release, discharge, and hold harmless Live to Dance • Dance to Live, its directors, officers, employees, volunteers, and all affiliated dance studios and their respective owners, instructors, employees, and volunteers (collectively referred to as "Released Parties") from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained by me or any third party arising from my participation in the dance program, including but not limited to:
   a) Negligence or other acts of the Released Parties;
   b) Any physical or emotional injury, illness, or condition that may result from my participation;
   c) Loss or damage to personal property; and
   d) Claims arising from the use of photographs, video recordings, or other likenesses taken during the dance program.
4. Indemnification: I agree to indemnify, defend, and hold harmless the Released Parties from and against any and all claims, demands, actions, or causes of action brought by any third party arising out of my participation in the dance program and any breach of this waiver and release of liability.
5. Consent to Emergency Medical Treatment: In the event of any injury or illness during my participation in the dance program, I authorize the Released Parties to obtain emergency medical treatment or services on my behalf if necessary. I understand that I am responsible for any medical expenses incurred as a result of such treatment.
6. Governing Law and Severability: This waiver and release of liability shall be governed by the laws of Florida, United States. If any provision of this agreement is deemed invalid or unenforceable, the remaining provisions shall remain in full force and effect.

I have carefully read this waiver and release of liability and fully understand its contents. I am aware that by signing this document, I am waiving certain legal rights that I or my heirs, executors, administrators, and assigns may have against the Released Parties. I voluntarily agree to be bound by its terms.
